Your business name (if applicable) and seller number Your contact information: email address and phone number A listing of all baby walkers you have posted Product images and product packaging images of all baby walkers you have posted (if applicable) .Images must include all sides of the product and product packaging for our review of product images or test reports to demonstrate compliance with registration card requirements (16 CFR Part 1130) Product images or test reports to demonstrate compliance with mandatory Sexual tracking label requirements (CPS 14(a)(5)) Product pictures or test reports demonstrating compliance with warning and hazard labeling requirements (ASTM F977-18 Section 8 or U.S. Title 16 Code of Federal Regulations Part 1216) (if applicable) all brochures and manuals for all infant walkers that you issue The Children's Product Certificate (CPC) for all infant walkers that you issue to which this policy applies A test report from a CPSC-accredited laboratory confirming that each item has been tested and complies with the above regulations and standards
